The brand new Discharge Ready Unit is a 36 bed unit dedicated to provide excellent patient care to those patients who are medically stable to discharge, but are experiencing a discharge delay to a community setting.

The unit utilizes a multidisciplinary approach of RNs, LPNs, CNAs, Care Management, Acute Therapies and Physicians to ensure that patients receive a holistic approach to optimizing their care as they transition out of the hospital.

Nursing staff aim to provide care to a wide variety of diagnoses, across multiple medical specialties. They will continue to provide wound care, pain management and patient teaching as appropriate to maintain or improve baseline functional status of the patient.

Background/Expertise:

  • Currently licensed in the State of Oregon to practice as an LPN.
  • Active BLS Certification required
