Responsibilities The Advanced Practice Clinician's primary role will be reviewing incoming clinical and medical considerations to ensure timely and appropriate placement within the hospital level of care. A secondary role is to assist in transitions of care and communication (warm-handoffs) from the hospital to adjacent programs and levels of care including residential, PHP, IOP, and or Outpatient Continuity Clinic providers. This person will work closely with interdisciplinary team members (i.e. Physicians, APCs, Nursing, Therapy, Admissions, receiving clinicians) to consistently and rapidly determine admission disposition and assist in smooth transitions of care.
