In-Field Registered Nurse (Care Manager)

Location: Westchester, NY (Field-Based)

About the Role:

Allegiant Home Care is seeking a compassionate and skilled Registered Nurse (RN) to join our
Concierge Care Model as a field-based Care Manager. In this role, you will be responsible for
providing direct clinical services to our clients in their homes. You will manage medication,
perform medical assessments, and provide hands-on care, including wound care, medication prepours and caregiver supervision. You will collaborate with the care team to ensure that all
aspects of the client's care are met with the highest level of professionalism and attention.

Key Responsibilities:
* Clinical Care: Provide skilled nursing care tailored to individual client needs. Tailor and
customize the plan of care to satisfy the client needs.
* Post-Operative Care: Assist with recovery following surgery, facilitate initial visit upon
discharge, perform wound care, adjust medications and clinical interventions to reflect
discharge instructions.
* Care Management: Develop and implement individualized care plans, ensuring clients
receive appropriate care at all stages of their recovery or chronic illness.
* End-of-Life Care: Provide compassionate care during end-of-life transitions, ensuring
comfort and dignity.
* Collaboration: Work closely with physicians, physical therapists, and other healthcare
providers to coordinate comprehensive care.
* Family Training: Provide caregiver training to family members, ensuring they are
equipped to support clients effectively.

Qualifications:
* Registered Nurse (RN) with active state licensure
* Experience in wound care, post-surgical recovery, and chronic care management
* Compassionate, client-centered approach to care
* Strong communication and team collaboration skills
