Pine Haven Care Center proudly serves Pine Island and the surrounding communities. Our 70-bed facility provides both short-term rehabilitation and long-term care for individuals requiring skilled nursing.

We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, helping them improve their mobility and overall quality of life. This role requires a strong understanding of anatomy, physiology, and medical terminology, as well as experience in various care settings including acute care, home health, and geriatrics.

Job Type: PRN
Wage: $39.75 - $44.13

Duties:


  • Conduct thorough assessments of patients' physical abilities and limitations.
  • Develop personalized treatment plans based on individual patient needs.
  • Implement therapeutic exercises and modalities, including trigger point therapy.
  • Educate patients and their families about treatment plans and home community care strategies.
  • Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Maintain accurate documentation of patient evaluations, treatments, and progress notes.
  • Collaborate with other healthcare professionals to provide comprehensive patient care.
  • Stay updated on the latest developments in physical therapy practices and techniques.Qualifications
  • A degree in Physical Therapy from an accredited institution.
  • Valid state licensure to practice as a Physical Therapist.
  • Strong knowledge of anatomy, physiology, medical terminology, and systems related to physical therapy.
  • Experience in clinic settings or acute care environments is preferred.
  • Familiarity with home health practices and geriatrics is a plus.
  • Excellent communication skills to effectively interact with patients and their families.
  • Ability to work independently as well as part of a multidisciplinary team.
