Position Summary

The Life Skills Professional is responsible for client support through direct interaction throughout shift; collaboratively and actively assisting and supporting the participants of the program in the development of Life/Recovery Support/Coping/Daily living skills. Facilitate and document psychotherapy groups, implementing the activities on the monthly calendar; promote and enhance the informed culture of the program and the establishment and attainment of goals identified in the individual Recovery Support Plan (RSP).

Essential Duties and Functions

  • Intervene in crisis and document all critical incidents. Review site's required documentation for completion and accuracy, i.e., observation checks, MARs, daily shift notes.
  • Support/assist the program participant with the development and maintenance of necessary daily life skills to live independently within their community, i.e., cooking, shopping, household chores, seeking employment and education opportunities, medication management, interpersonal relations, utilizing community resources, vocational, budgeting, banking, accessing transportation, meal preparation, etc.
  • Works in cooperation with other agencies in the service delivery system to ensure coordinated, continuous and effective services for the rehabilitation of the participants.
  • Monitor participant medication as prescribed (after completion of Medication Administration Training), and complete all corresponding documentation.
  • Complete required documentation comprehensively and accurately, i.e., observation checks, daily residential shift notes, mileage logs, incident reports (if applicable), and vehicle incident reports (if applicable).
  • Provide emergency medical procedures as needed, including Basic First Aid, and CPR (after completion of CPR Training).
  • Facilitate and document educational groups and any other significant contact.
  • Conduct, facilitate and document fire drills.
  • Read daily logs and document in logs at the time of shift and end of each shift.
  • Complete intake packets and inventory upon every admission into the program and discharge.
  • Escort clients to appointments and shopping as designed within the program schedule and or according to client treatment plan.
  • Conduct daily documentation and filing of Sign In logs, Refrigerator logs, Shift logs, and monitoring of clients movement.
  • Attend scheduled/mandatory training as outlines in CBH, DDAP, and employee's individual training plans.
  • Responsible for attending supervision, clinical meetings, monthly staff meetings, and mandatory trainings to remain current with best practices.
  • Operate agency vehicles in accordance with established policies and procedures.
  • All other duties as identified by the Program Director/Assistant Director/Lead LSP

Qualifications and Skills

  • Possess High School Diploma/GED
  • 2 years of experience in the social service field. CPRP, CPS, WRAP trained, a plus.
  • Must possess a current valid State Driver's License.
  • Flexible schedule
  • Independent thinking and decision making
  • Computer knowledge (Electronic Health Records experience a plus)

Working conditions

While performing the duties of this job, employees are frequently required to stand, walk, lift, use hand to find, handle feel, reach with hands and arms, climb or balance, stoop, kneel, crouch, crawl, talk, listen, taste, and smell. The job also requires employees to lift and/or move objects up to 50lbs. The individual must be able to manager stairs on a frequent basis.
