This is a Temporary, Non-Benefited position that starts April 20, 2025 and ends on October 19, 2025 Job Summary: The Supervisor-Disease Management supervises and oversees the Disease Management (DM) program and assigned staff. Responsibilities include staff education, performance analytics and improvement (PI), establishing collaborative relationships between DM, Managed Care, clinical leadership, physicians and staff to promote care coordination of patients with chronic disease. Investigates grievances and conducts facilities audits. Performs other duties as needed.
Education and Experience: Bachelor's Degree in healthcare or related field required. Minimum three years experience in a clinical and/or outpatient setting required, preferably Quality Management experience. Experience may be considered in lieu of Degree.
Knowledge and Skills: Knowledge: Applies acquired knowledge within the LVN scope of practice to mentor staff in identification of complex cases and problem solving based on quality program indicators, and departmental organization and policies. Supervisory principles and practices. Essential Technical/Motor Skills: Able to keyboard 40 wpm. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Operate/troubleshoot basic office equipment required for the position. Essential Interpersonal Skills: Able to relate and communicate positively, effectively, and professionally with others; work calmly and respond courteously when under pressure; collaborate and accept direction. Essential Mental Abilities: Able to communicate effectively in English in person, in writing, and on the telephone; think critically; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. Essential Sensory Abilities: Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.
Licensures and Certifications: Current Valid Driver's License required. Current California State Licensed Vocational Nursing (LVN) license required. Basic Life Support (BLS) certification required.
