POSITION SUMMARY: Initiates and maintains all patient medical records, discharge records, and outpatient records. Preps and analyzes all medical records for completeness. Locates and eliminates duplicate medical records for the paper-based and electronic medical record. Primarily responsible for quality assurance of all documentation received in the H.I.S. department. Reviews, prepares and scans patient health information from a paper format into the document imaging software Content Manager and/or Cerner data repository.

Ensures 98% - 100% accuracy and quality of scanned documents. Maintains scanning equipment on daily basis. Participates in inter- and intra- departmental document imaging operations and assists in the maintenance of the internal HIS document process flows. Assesses electronic documentation for patient care facilitating delivery of healthcare services.

Updates patient demographic data and other information in multiple software applications utilized in the Health Information Services department related to maintaining the Master Patient Index and electronic medical record. Electronic documentation functions are maintained in conjunction with HIS Director, H.I.S. Operations Manager and clerical staff seven days a week.



Requirements

MINIMUM QUALIFICATIONS:

EDUCATION: High school diploma required; AS degree in Office Administration, Health Information Management (or related field), or equivalent experience preferred.

CERTIFICATION/LICENSES: RHIT and Notary Public Certificates preferred.

SKILLS:



  • Ability to work on several projects simultaneously in a fast paced environment
  • Computer database/network search capability, process management for electronic document distribution, workflow monitoring, keyboarding and word processing skills preferred; Medical terminology, Joint Commission standards familiarity
  • Ability to interact effectively with hospital staff at various levels in the organization and demonstrate excellent listening, written and oral communication skills, including English usage, grammar, punctuation and style preferred
  • Ability to learn and use new software to include document imaging and word processing applications
  • Demonstrates strong organizational skills with the ability to manage several priorities at one time and to work in a team setting.


EXPERIENCE: Two years in office or healthcare environment required. Experience with computer applications in healthcare required or successful completion of required course work in Health Information management.

-Responsible to: Operations Manager, Health Information Services

  • Bloodborne pathogen A
  • General office environment. Knowledge of general safety standards. Exposure to hazards from electrical/mechanical/power equipment.
