Summary

Screens/evaluates referrals for admission to determine eligibility. Secures physician approval for admission. Verifies insurance coverage information. Maintains optimum unit census through coordination of admissions and discharges.

Job Duties

  • Receives calls from referral sources and initiates referral process to begin intake.
  • Reviews patient medical record for test results, therapy evaluations, pre-existing conditions, pre-morbid level of functioning, and potential discharge plan post rehab stay.
  • Verifies patient's insurance and follows through with the insurance plan benefits, and if pre-authorization is required, will complete the process and document within authorization/certification section of the medical record.
  • Ensures all regulatory requirements for admission are met prior to approving the patient for admission to facility.
  • Maintains bed management of patients in order to place patient in most appropriate environment.
  • Collaborates with case management staff and/or physicians regarding anticipated discharge from facilities.
  • Provides facility staff with current, accurate information on new referrals prior to their transfer to unit.
  • Identifies quality issues and participates in quality management activities.

Minimum Qualifications
  • Master's Degree or Bachelor's in Physical Therapy or
  • Master's Degree Occupational Therapy or
  • Master's Degree Speech Therapy
  • 2 years Recent experience in an inpatient rehabilitation facility or
  • 1 year Admission/liaison Experience and
  • 1 year Acute or long term care setting
  • Excellent problem solving skills.
  • Knowledge and understanding of inpatient rehabilitation admission criteria.
  • Strong clinical data analytic skills measuring process and outcomes in healthcare.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire or
  • PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire or
  • OT - Licensed Occupational Therapist - State of Pennsylvania Upon Hire or
  • SLP - Licensed Speech Language Pathologist - State of Pennsylvania Upon Hire

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
