Job Summary

The role of this position is to provide support, education, referrals, and resources to help navigate pediatric hematology-oncology patients and families through transitions of care within the UC Davis Children's Hospital and the UC Davis Comprehensive Cancer Center.

Under general direction and team collaboration with inpatient and outpatient providers, the Pediatric Hematology-Oncology Patient/Family Navigator conducts needs assessments; identifies barriers to care for patients and families; coordinates efforts to reduce barriers by connecting patients and families to UCDH and community services; and facilitates logistical transitions between outpatient and inpatient care. #CA-SB

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • Minimum 2 years professional experience in patient navigation or medical or social services field helping patients and families access services and benefits in the community (schools, hospital/clinics, mental health care, human services, etc.)
  • Experience working with the general public and community agencies in a collaborative and professional setting.
  • Experience and comfort working with multi-generational families, including non-traditional family structures.
  • Customer service and cultural humility orientation.
  • Must be skilled in relating to a multi-disciplinary team and able to promote and engage in relationship building with the team and with patients, families and caregivers.
  • Strong professional boundaries, and ability to work collaboratively with team members to address patient needs utilizing a team.
  • Excellent communication skills, including positive interpersonal skills working with a diverse clientele and community-based organizations.
  • Proficient computer skills, including Microsoft Word, Excel and Power Point software programs.
  • Ability to curate computer searches related to diagnosis; work with SOS team to identify and update trusted internet search resources.
  • Efficient with time and skills, able to produce significant work output with minimal direction.
  • Attention to detail and delivery of work that is accurate and complete.
  • Ability to organize and prioritize multiple tasks according to program needs and established deadlines.

Preferred Qualifications

  • Medical Assistant Certification.
  • Bachelor's degree, preferably in the field of social work (BSW), child development, health care, psychology, or human services.
  • Medical/health care experience, preferably in an oncology setting.
  • Basic knowledge and experience with EPIC.
  • Bilingual fluency in Spanish strongly preferred.
  • Familiarity with cancer related information, such as diagnoses, medical terminology, support services and survivorship.
