Senior Intake Care Coordinator

Location: Mariposa County, California; Merced County, California; Monterey County, California; San Benito County, California; Santa Cruz County, California

We have an opportunity to join the Alliance as a Senior Intake Care Coordinator in the Care Management Department.

There are 2 opportunities available, 1 for our Pediatric Care Management and California Children's Services team and 1 for our Behavioral Health Care Management team.

WHAT YOU'LL BE RESPONSIBLE FOR

Reporting to the Pediatric Care Management and California Children's Services Manager or the Behavioral Health Care Coordination Supervisor, this position:



  • Acts as Team Lead and provides technical leadership and training to Intake Care Coordinators - Care Management
  • Assists team with the coordination of health care for Alliance members
  • Assists with special projects, team operations, and supports leadership to implement program improvements that strengthen internal operations, member access, and health outcomes
  • Establishes and maintains effective working relationships with Provider offices, County departments, and other community agencies related to care coordination for members

  • Knowledge of:

    • Managed health care and health care systems
    • Customer service principles and practices
    • Care management principles and practices
    • Community care resources within the Alliance's Service Area Counties
    • Diverse needs of the Medi-Cal population


  • Ability to:

    • Lead, train and mentor staff
    • Perform care coordination work associated with the most complex member cases
    • Understand and communicate the workings of the Alliance, particularly the Utilization Management and Complex Case Management, Care Coordination and Member Services departments
    • Competently navigate computer systems
    • Work independently with minimal supervision
    • Identify, maintain, and protect sensitive HIPAA information (Personal Health Information) and following procedures to ensure the security of such information


  • Education and Experience:

    • High school diploma or equivalent
    • Four years (or an Associate's degree in health, social services or a related field or Medical Assistant certification) experience, which must have included a minimum of two years of experience as an Alliance Care Coordinator; or a combination of education and relevant work experience may be qualifying




OTHER INFORMATION



  • We are in a hybrid work environment and we anticipate that the interview process will take place remotely via Microsoft Teams.
  • While some staff may work full telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected.
  • In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process.


The full compensation range for this position is listed by location below.

The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferrable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location).

Typical areas in Zone 1: Bay Area, Sacramento, Los Angeles area, San Diego area

Typical areas in Zone 2: Fresno area, Bakersfield, Central Valley (with the exception of Sacramento), Eastern California, Eureka area

Zone 1 (Monterey, San Benito and Santa Cruz)
$28.97 - $46.35 USD
Zone 2 (Mariposa and Merced)
$26.36 - $42.18 USD

Available for all regular Alliance employees working more than 30 hours per week. Some benefits are available on a pro-rated basis for part-time employees. These benefits are unavailable to temporary employees while on an assignment with the Alliance.
