Responsibilities

Hours for this role are either a 0.8 or 1.0 TE working Friday, Saturday, Sunday and Monday

As the AID Patient Intake Coordinator, you will be the first point of contact for patients seeking mental health services. Your primary responsibility will be to handle both external and internal requests for information and services across all levels of care. You will conduct Mental Health Screens over the phone, schedule vital assessments and appointments, and provide essential crisis support. Key Responsibilities:
  • Information and Assessment: Review and assess callers' needs, linking them with the appropriate services and information about PrairieCare.
  • Crisis Management: Triage crisis calls, assess safety, and develop safety plans while providing county crisis resources as needed.
  • Scheduling: Schedule Admission Diagnostic Assessments and intake appointments for both program and outpatient clinic services.
  • Referral Coordination: Facilitate internal and external direct referrals to our services, consulting with physicians as necessary to ensure smooth transitions into our programs.
  • Customer Relations: Provide exceptional customer service in line with PrairieCare's policies, ensuring a supportive and informative experience for all callers.
  • Online Engagement: Monitor online channels for patient inquiries through our website and follow up with patients as appropriate.
  • Needs Assessments: While not the primary function, you may occasionally be asked to complete Needs Assessments to support our patients further.

The nature of this position is highly variable. At times individuals in this role may be asked to complete Needs Assessments. While this is not the primary function of this role, staff may be asked to provide support in this manner.


Qualifications

  • Minimum Requirement: High School Diploma or equivalent is required.
  • Preferred: Bachelor's degree or two (2) years of applicable experience in the behavioral health field, providing a solid foundation for your role.
  • Expertise in Mental Health: Strong knowledge of DSM-5 criteria, with demonstrated interviewing and assessment skills that allow you to effectively evaluate and support our patients.

  • Cultural Competence: Oriented and educated in gender, cultural differences, and age-appropriate care for psychiatric and chemical dependency services, ensuring that every patient feels respected and understood.

  • Effective Communication: Exceptional communication skills, both verbal and written, enabling you to convey important information clearly and collaborate effectively with clinicians and team members.

  • Tech-Savvy: Proficiency in Microsoft Office products, including Word and Outlook, to manage documentation and communication efficiently.

  • Adaptability: Ability to smoothly and rapidly shift between dissimilar tasks, demonstrating flexibility in a fast-paced environment.

  • Calm Under Pressure: Proven ability to react quickly and calmly as part of a team in volatile and potentially emergency situations, ensuring the safety and well-being of our patients.

  • Decision-Making Skills: Strong critical thinking abilities that empower you to make effective decisions while navigating stressful circumstances.

Pay range for this position is $22.06 to $28.84 depending on education and experience.
