Description

At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

Position Overview:

Join Healthcare in Action's Street Medicine Team as a Peer Navigator/Housing Navigator, supporting the unhoused populations in Los Angeles. This position involves working collaboratively with the Street Medicine Team and Care Management to provide vital community support to those in need.

Responsibilities:



  • Serve as a peer navigator for the unhoused population, offering guidance, support, and advocacy.
  • Collaborate with the Street Medicine Team and Care Management to coordinate and provide necessary healthcare and social services to individuals experiencing homelessness.
  • Assist in connecting individuals to housing resources, navigating housing systems, and providing ongoing support in securing stable housing solutions.
  • Build and maintain relationships with community partners and resources to enhance support services available to the unhoused population.
  • Advocate for the unique needs and challenges faced by individuals experiencing homelessness within the healthcare system and other relevant sectors.
  • Maintain accurate records and documentation of interactions and services provided.


Qualifications:



  • Experience working with unhoused populations, understanding their needs, challenges, and the Los Angeles homeless landscape.
  • Strong communication, interpersonal, and advocacy skills. - Knowledge of local housing resources, social services, and healthcare systems.
  • Ability to work effectively in a multidisciplinary team and independently.
  • Empathy, cultural competence, and a non-judgmental approach to supporting diverse populations.


Requirements:



  • Bachelor's degree in social work, public health, or a related field preferred.
  • Relevant experience in social services, community outreach, or healthcare support roles.
  • Familiarity with Los Angeles area resources and services for the unhoused population.
  • Valid driver's license and reliable transportation. -
  • Willingness to work flexible hours, including evenings and weekends.

  • This position is a patient-facing role.



  • Employees may be eligible for differential pay.


  • Other duties as assigned.


Tuberculosis Screening Policy
