Responsibilities
The Family Access Coordinator is a customer focused, solution oriented, consultative and case management approach to providing support to family and chosen family members throughout the continuum of Family Services. The Family Access Coordinator will offer a starting point and ongoing navigation for family and chosen family members who are in need of support and services for their own journey, regardless of their loved one's path to care. Utilizing a comprehensive understanding for the Family Services system the coordinator will, on an on-going basis, guide family members, chosen family members and other concerned persons to the best solution(s) based on the individual's needs, urgency, financial abilities, and the Family Services model of care. Family Access Coordinators are the gateway to family services. They provide initial guidance, support and empathy while screening for early services to prospective participants. They also retain regular contact with those receiving Family Services to ensure each individual progresses through services as needed throughout their healing journey. Always, they provide a clear path through the menu of services available for family members. This position is responsible for screening, managing, and converting callers to the appropriate program(s) in addition to managing the process from the initial conversation through the registration process and along any additional services as appropriate. The Family Access Coordinator will also often collaborate with multiple departments within the organization to ensure a seamless, timely flow of a family member or loved one through services, ensuring expectations of the customer are met and exceeded.
