Job Summary |
The College of Pharmacy's Clinical Affairs and Outreach Administration is seeking an
ICARE Ambulatory Care Pharmacist I to join its interdisciplinary healthcare team. This position collaborates with healthcare professionals at Auburn University and East Alabama Health as part of a
Transitions of Care program. The primary goal of this program is to reduce hospital readmission rates by providing coordinated, patient-centered care during the transition from hospital to home. |
Essential Functions |
- Provides medication therapy management consultation and other pharmaceutical care services in a manner that facilitates the achievement of both a high level of patient satisfaction and the attainment of organizational care and service initiatives.
- Reviews prescribed medications and dosages to ensure that therapy is safe and effective for the patient's conditions and that all conditions have a corresponding treatment plan if indicated.
- Monitor and disseminate information concerning program outcomes.
- Identify, develop, and implement program improvements through a quality improvement initiative.
- Compile reports provided to grant funding agencies
- Communicate with other healthcare providers and disciplines to coordinate patient care when transitioning in and out of the hospital.

Minimum Qualifications
Minimum Qualifications |
- Doctor of Pharmacy degree (PharmD)
- 1 year experience in pharmacy practice

Minimum Skills, License, and Certifications
Minimum Skills and Abilities |
- Knowledge of Federal and State guidelines and laws related to the practice of pharmacy in the state of Alabama
- Excellent communication and patient care skills
- Strict adherence to patient privacy laws (HIPAA)

Minimum Technology Skills |
- General office equipment; electronic medical record, medical testing and monitoring equipment (glucometer; stethoscope, etc.)

Minimum License and Certifications |
- Licensed pharmacist or eligible for licensure in the state of Alabama
