Responsibilities

We are looking for a Social Worker to join our team within the Adult Intensive Outpatient Program (IOP). If you are passionate about making a meaningful difference in the lives of patients facing psychiatric challenges and thrive in a collaborative environment, we invite you to apply for this rewarding opportunity! As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient's primary treatment team to ensure a cohesive and effective plan of care

Psychosocial Detective: Use your keen assessment skills to uncover the unique stories and needs of our patients. Conduct thorough psychosocial and suicide risk assessments that help us understand the whole person.

Treatment Plan Architect: Collaborate with patients and their care team to build personalized treatment plans that are as unique as they are! You'll be the mastermind behind creating and updating these plans to ensure everyone is on the same page.

Care Coordinator Extraordinaire: Be the glue that holds everything together! Coordinate care with outpatient providers during admissions and discharges to ensure a smooth transition for our patients.

Psychoeducational Group Leader: Bring your creativity and knowledge to life by leading engaging psychoeducational and therapeutic groups that empower patients to thrive!


Qualifications

  • Master's Degree in Social Work and licensure in the State of Minnesota required (LGSW).
  • Experience in healthcare setting preferred
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word and Outlook
  • Proficient in Microsoft Office Products including Word, Outlook, and Teams.
  • Able to effectively and efficiently communicate in written and verbal form.
  • Ability to smoothly and rapidly shift between dissimilar tasks.
  • Ability to respond appropriately in times of mental health crisis.

Compensation for this role ranges from $29- $34 per hour depending on education and experience.

An individual's pay rate within the posted range is determined by several factors, including skills, experience, education, and market conditions. Additionally, we consider internal equity to ensure fairness within our organization.
