Position Title: Housing Case Manager

Job Summary: Responsible for managing The Salvation Army's of Pekin 360 Life Center Emergency Homeless Shelter Rapid Rehousing Assistance Program in Pekin and Tazwell County. This person will focus on accessing stable housing to provide positive housing solutions for persons transitioning from interim shelter to community-based living. Position includes providing resource referrals, screening participants for eligibility with various funding partners, and administering assistance according to agency and funder standards.

Reports to: Social Services Manager

Supervises: N/A

Salary Grade: 205

Pay Range: $19.97 - $23.96

Qualifications:

Candidates must successfully pass the pre-employment background inquiry and annual motor vehicle check with a valid driver's license.

Education:

Bachelor's degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.

  • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
  • Ability to establish policies and procedures; holding others accountable to the same.
  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Demonstrated awareness of budget resources and cost control, as well as planning and administering budgets.
  • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, and case management databases.

Essential Responsibilities:



  • Provide case management services to guests in the Pekin Shelter to include, but not limited to: reviewing intake material for accuracy, assessing client needs, case plan development and compliance, act as liaison to community resources and provide referrals to appropriate agencies.
  • Determine program participant eligibility and conduct client intakes and evaluating the level of need, completing all appropriate assessments and paperwork, gathering documentation.
  • Ensure that all clients have access to a case manager through appointments and walk ins.


    • Establish first contact within 3 days
    • Meet with clients weekly or as otherwise determined
    • Schedule and conduct discharge planning
    • Perform follow-up contact at regular intervals with discharged clients.


  • Maintain written and electronic case files with all relevant client information along with advocacy, financial services, referrals, payment arrangements, and outcomes.
  • Maintain and submit monthly statistics in a timely order as required for The Salvation Army and other grants received.
  • Submit all electronic records in a timely manner on all internal and external databases of Teams, SIMS, HMIS, and others as applicable for grant requirements.
  • Collects, compiles, and reports statistical data to the Social Service Operations Manager.
  • Develops and fosters local relationships with other services/ healthcare/ educational/ municipal entities to ensure effective local support and referral sources.
  • Attending staff meetings and community events as necessary.
  • Participate in renewal of RRH contract and funding plan requests.
  • Other duties as assigned by the Social Services Manager.
  • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
  • Maintain compliance in The Salvation Army's "Safe from Harm" Certification.
  • Travel will be required.

Physical Demands/ Work Environment:

This Position is required to do basic business and office work. In order to successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit and use hands. Individuals must be able to regularly lift up to 20 pounds and occasionally lift up to thirty pounds. The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position.
