Summary The Clinic Specialty Pharmacist acts as an extension of ambulatory clinic teams to support patient access to high touch, complex medication therapies. This position is based in a specialty medical clinic setting. The Clinic Specialty Pharmacist functions in a multi-skilled environment, which requires the application of teamwork, leadership, and flexibility. This position is responsible for general supervision of Specialty Pharmacy Care Coordinators and for the operation and services provided in the specialty pharmacy clinic. Provides clinical consultation, medication education, chronic disease state management, medication reconciliation, and adherence counseling for specialty patients. Plays an integral role with interdisciplinary teams including pharmacists, nurses, medical assistants, and providers to facilitate patient care. Furthers the development, implementation, and maintenance of programs that promote optimal patient outcomes.

Baptist Health is looking for a Clinic Specialty Pharmacist - Endocrinology to join our team at 521 Robinbrooke Blvd, Elizabethtown, KY!
- Serves as a primary point of contact with patients related to specialty medication needs and demonstrates empathy and compassion to patients and caregivers.
- Enrolls specialty pharmacy patients into medication management programs by assisting with recruitment of patients in clinic.
- Provides initial clinical medication counseling and regular patient reassessments via in person, telephonic, or video encounters to ensure appropriateness of specialty therapy and ongoing therapy management.
- Performs medication therapy management, which may include coordinating medication adjustments to optimize patient medication regimens and improve clinical outcomes.
- Refers appropriate patients to financial counseling for copay assistance through manufacturer or charity programs.
- Collaborates with providers and clinical staff to integrate pharmacy services and coordinate care with eligible patients.
- Communicates with physicians and other clinicians as necessary to facilitate exemplary service for new and existing Baptist Health specialty pharmacy patients.
- Provides limited/indirect supervision for Specialty Pharmacy Care Coordinators and timely response to clinical escalations.
- Demonstrates commitment to the quality, philosophy, and values of Baptist Health by maintaining compliance with established policies and procedures, organizational objectives, process improvement, and safety standards.
- Participates in ongoing trainings related to role, including but not limited to disease state overviews, clinical guidelines, pharmacology, REMS programs, etc.
- Engages in personal professional development to stay updated with pharmacy trends related to role and practice setting.
- Communicates and represents pharmacy effectively throughout all levels of the organization internally and to all external partners.
Minimum Requirements
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
- Kentucky Board of Pharmacy license is required.
- 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
- Experience in ambulatory clinical pharmacy practice preferred.
- Specialty disease state experience preferred.
