JOB DESCRIPTION: SUMMARY OF POSITION This position will work within the Coordinated Opioid Recovery program (CORe) in Hendry and Glades Counties and will primarily work out of Labelle. Services will be provided to adults with opioid and other substance use disorders who have had an overdose reversal with naloxone or acute opioid and substance use and who are ready to begin recovery. Services will be community-based with some services provided in office. This position will work as a part of the interdisciplinary CORe team to partner with EMS (community paramedicine), law enforcement, and the local hospital system to identify individuals in need of services in order to provide seamless continuity of care that supports each individual's recovery journey. The team will also connect with community partners through meetings, informal connections, community fairs, and presentations to expand program reach and build community trust. Provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. This role will ensure continuity of care with clients through case management services as well as link clients and their families to needed resources to help to improve their quality of life. A variety of clinical training opportunities are available based on program and interest. ESSENTIAL DUTIES & RESPONSIBILITIES
KNOWLEDGE, SKILLS & ABILITIES
QUALIFICATIONS Education Level Bachelor's degree required. Good organizational, time management, written and verbal communication skills. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries. Years of Experience Certification/Licensure Community-based program case managers, who either take crisis calls and/or home visits, must have a valid FL driver's license with a good driving record and current auto insurance.
