The Occupational Therapist is a qualified professional who directs, supervises, evaluates, and provides Occupational Therapy services to patients in the home according to State Law and regulations under the direction of the attending physician.

Responsibilities

  • 1. Comprehensive evaluation of the patient's level of function and development of the occupational therapy plan of care with consideration of loss/impairment of abilities to complete activities of daily living.
  • 2. Selects and administers diagnostic and therapeutic techniques and materials.
  • 3. Assists in determining aspects of the plan of care for delegation to home health aides and assesses all factors pertinent to patient safety.
  • 4. All documentation is completed within 24 hours in the EMR.
  • 5. Begins discharge on admission and discharges patients when goals are met.
  • 6. Participates in case conferences as determined by the agency and documents.
  • 7. Instructs patient, family, home health aides, COTAs, and other clinical staff regarding the patient's treatment plan with follow-up.
  • 8. Coordinates care with patient, caregivers, RN Case Manager, and other disciplines involved in the patient's plan of care.
    • 8.1 Coordinates and manages the care of patients receiving services in their home.
  • 9. Identifies the need for equipment and supplies and instructs the patient and caregiver accordingly.
  • 10. Responsible for ensuring insurance authorization prior to visiting the patient as appropriate.
  • 11. Participates in staff meetings, in-service programs, and other meetings as requested.
  • 12. May perform other duties as assigned.

Qualifications

EDUCATION

Bachelor of Science (or higher) in Occupational Therapy.

EXPERIENCE

Minimum of two (2) years of clinical experience in Occupational Therapy in an acute or sub-acute care setting. Home care experience is preferred.

LICENSURE

Occupational Therapist with valid (or eligible) Connecticut license.
A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day.
CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

SPECIAL SKILLS

Excellent written and verbal communication.
Ability to work collaboratively with multidisciplines.
Must be adaptable, flexible, able to learn quickly, use critical thinking skills, and oversee COTAs.
Strong computer skills.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs.
Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
