Job Summary and Responsibilities

Acentra Health is looking for a HCBS/LTSS Assessments Policy Director to join our growing team.

Job Summary:

We are seeking a highly experienced and policy-driven expert to lead and support initiatives related to Home and Community-Based Services (HCBS) and Long-Term Services and Supports (LTSS) assessments. This role will ensure compliance with federal and state regulations, promote person-centered practices, and drive quality improvement in assessment processes across Medicaid-funded programs. This role will also act as a thought leader, leading our transformation of assessment space in the industry.

Responsibilities:

  • Interpret and apply CMS regulations related to HCBS and LTSS, including the HCBS Settings Rule and PASRR requirements.
  • Monitor and support compliance with Statewide Transition Plans (STPs) and CMS assurances
  • Design and refine assessment tools and workflows for eligibility and service planning.
  • Lead initiatives to improve assessment accuracy, equity, and efficiency.
  • Collaborate with clinical teams, IT, and data analysts to integrate assessments into broader care coordination systems.
  • Analyze assessment data to identify trends, gaps, and opportunities for improvement.
  • Support continuous quality improvement (CQI) efforts across HCBS and LTSS programs.
  • Serve as a subject matter expert (SME) for internal teams, state agencies, and external partners.
  • Facilitate training and technical assistance for assessors, providers, and care managers.
  • Represent the organization at national conferences and policy forums.
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications

  • Master's degree or higher in Public Health, Social Work, Health Policy, or related field
  • 5+ years of experience in Medicaid, HCBS, LTSS, or related policy and assessment work
  • 3+ years' experience analyzing state Medicaid operations for assessments
  • 3+ years preparing correct action plans to ensure they are conflict-free, culturally competent, and aligned with person-centered planning principles
  • Deep understanding of CMS regulations, person-centered planning, and assessments best practices
  • Strong analytical, communication, and project management skills
  • Experience working with diverse populations, including individuals with disabilities, behavioral health needs, and aging adults

Preferred Qualifications

  • Experience with CMS Technical Assistance Centers or state Medicaid agencies
  • Familiarity with electronic assessment platforms and interoperability standards
  • Background in clinical psychology, social work, or nursing is a plus
