$21.98 - $24.73 / hour Position Summary: Case Aides report to the FFT-CW Supervisors. They provide culturally competent case management and advocacy to families in the FFT-CW Program in compliance with ACS policies and procedures. The Case Aide is part of a clinical team that comprises of an FFT CW Program Director, a Supervisor, and up to four Therapists or Interventionists. The Case Aides' duties are performed in support of, as well as in coordination and collaboration with the FFT-CW Therapists, Interventionists, and Supervisors. The primary duties of the case aide are to assist and support the case planners in collecting critical information, such as educational and medical, and linking families to appropriate community resources. Based on Case Planners' assessments, the Case Aide will provide families with concrete case management support and advocacy services to address families' need for support in areas such as Public Assistance, housing, school-related issues, and referrals for community-based services such as parenting classes, mental health services, job readiness programs, after school activities, summer camps, and other pro-social activities, etc. FFT-CW (Child Welfare) is a therapeutic program in our Prevention continuum of services. It is a home-based intensive therapeutic model for families in the Bronx with multiple risk factors, where there has been physical abuse and/or neglect of a child/ren aged 0-17. It is an evidence-based family therapy model that focuses on engaging with the family's broader social environment and develops each family's social and community networks. FFT-CW has two service tracks within the model:
Essential Duties: General responsibilities of the position of a Case Aide will be to support the work of the FFT-CW Therapists and Interventionists by assisting families with concrete needs and services, educational advocacy, and referrals to support services to alleviate barriers to their participation in treatment. Bachelors of Social Work (preferred)
Associates (required) Experience
Minimum one year of related experience working with children, adolescents, and/or families preferably in a child welfare, juvenile justice, or mental health settings. (required)
