GENERAL PURPOSE OF THE CARE COORDINATOR JOB Reporting to the Manager/Supervisor, this Care Coordinator role will coordinate with providers and Integrated Care Nurses to provide ongoing support and communication to End Stage Renal Disease (ESRD) patients. The Care Coordinator acts as a single point of contact to coordinate resources for DaVita IKC patients. ESSENTIAL DUTIES & RESPONSIBILITIES of the CARE COORDINATOR: ESSENTIAL FUNCTION - % OF TIME 1. Performs outbound calls and responds to inbound calls from members, providers, and Village resources. Under guidance of Manager, completes non-clinical pathways & screenings; escalates to Integrated Care Nurse as needed. - 50% 2. Helps identify eligible resources for members and works with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in eligible programs - 30% 3. Assists with the referral process & coordinates provider appointments on behalf of DaVita IKC members - 15% 4. Other duties as assigned - 5% Total Allocation of Time = 100% The above statements describe the general nature and level of work being performed by individuals employed in this Care Coordinator position.
