Intake Coordinator

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being. We are currently seeking a Intake Coordinator to work in our AnchorPoint Children's Crisis Residence Program.

Position Summary:

The Intake Coordinator will provide essential support to ensure the smooth and efficient operation of admissions to the AnchorPoint Children's Crisis Residence. This position functions as a part of a multidisciplinary team, under the direct supervision of the Program Director. The Intake Coordinator plays a critical role in assessing initial eligibility, coordinating information gathering, and facilitating smooth transitions into the program, all in alignment with NYS OMH guidelines and the program's family-centered, trauma-informed philosophy.

Essential Functions and Responsibilities:

1. Fulfills training responsibilities according to Children's Home of Poughkeepsie, NYS Justice Center, and OMH regulations.
2. Is familiar with and adheres to all agency policies and procedures as outlined in the Children's Crisis Residence's Policy & Procedure Manual and Employee Handbook.
3. Responsible for managing all aspects of the referral and pre-admission process.
4. Serve as the sole point of contact for all incoming referrals from hospitals, schools, community mental health providers, mobile crisis teams, and families, and other referral sources, ensuring timely, clinically appropriate, and regulatory-compliant admissions to the crisis residence.
5. Receive, log, and track all referral calls, emails, and documentation in the program's electronic health record (EHR) and referral tracking system.
6. Provide timely acknowledgment of all referrals and referral determinations and communicate expected response timelines to referring parties.
7. Maintain an accurate and up-to-date referral log, including disposition of all referrals (admitted, declined, diverted, waitlisted).
8. Conduct structured telephone or telehealth screenings with referral sources, parents/guardians, and when appropriate, the child or adolescent to assess preliminary eligibility for crisis residence services.
9. Review clinical documentation including psychiatric evaluations, prior treatment records, safety assessments, and diagnostic information to determine program appropriateness.
10. Apply NYS OMH admission criteria, Level of Care guidelines, and AnchorPoint program-specific eligibility

Schedule: Monday - Friday 8am-5pm or 9am-6pm, with rotating on-call responsibilities, Saturday-Sunday OFF.

MINIMUM Qualifications:

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field required.
• Excellent phone skills.
• Minimum of two (2) years of experience in a mental health, child welfare, or behavioral health setting, preferably serving children and adolescents.
• Must be comfortable working with the public, in person and by phone.
• Demonstrated strong computer and word processing skills.
• Valid NYS Driver License in good condition as determined by the Agency
• Must complete and pass a pre-employment physical, PPD and drug screen and an annual physical and PPD.

PREFERRED Qualifications:

• Experience with Medicaid/managed care and other types of client billings preferred.
• Master's degree in Social Work (MSW), Mental Health Counseling, or a related clinical field preferred.
• Experience with crisis services, inpatient psychiatric care, or residential programming strongly preferred.
