Description Dementia Health Coach - GUIDE Program Schedule:Full-Time | 40 hours/week | Monday-Friday, 8:00 AM-4:30 PM Overview The GUIDE Program is expanding, and we are seeking a dedicatedDementia Health Coachto join our multidisciplinary Dementia Care team. This role is ideal for someone deeply committed to improving the lives of individuals living with dementia and their caregivers. As a Health Coach, you will provide compassionate peer support, help families navigate complex care needs, and connect them with meaningful community resources-including respite care. You will play a central role in care coordination, ensuring that each patient-caregiver dyad receives holistic, person-centered support. What You'll Do
