As a SocialWorkerMSW, you will work with a care team toprovidesupportive services to patients and their families. Assists Director to ensure all staffisprovidingquality care to patients and families from admission through discharge and aftercare. Primary Duties:

• Assesses the patient's and family's psychosocial needs as related to the patient's terminal illness. Consults and collaborates with the interdisciplinary team in developing and implementing the comprehensive care plan.

• Documentsproblems,psychosocialassessment,appropriate goals, care provided, interventions and patient, family, and caregiver response to each intervention. Assesses and refersforongoing bereavement services.

• Supports the patient unique cultural and spiritual beliefs with referrals to clergy or chaplain asrequested. Enhances the responsiveness of the environment by connecting the patient,familyand caregivers with community resources as needed.

• Providesinterventions for specific symptom relief (fear, grief, depression, anger, etc.).Screens for psychopathology and educates/intervenes accordingly.

Qualifications:

• Master's degree in social work.

• LMSW (Licensed Master Social Worker).

• Healthcare experiencepreferred.

• Experience inhomehealth carepreferred.

• Completed hospice training programpreferred.
