Position Summary

The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase. The job duties vary with the particular setting or assignment. Transplant Nurse Coordinators can be assigned to kidney, pancreas, liver and/or small bowel programs. Clinical Nurse Consultant II is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Clinical Nurse Consultant II provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice. The CNCII strives to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice

Duties & Responsibilities

  • Evaluates patients at the appropriate UI Health site of care.

  • Coordinates care for transplant recipients from referral and /or until waitlisted, all potential recipients with living donors and all living donor patients.

  • Ensures that all pre-transplant patient coordination and patient education is documented and is in compliance with UNOS, CMS transplant program guidelines and UI Health policies/procedures.

  • In collaboration with the provider, orders all testing for evaluation as needed and reviews results with the provider in a timely manner following UNOS and CMS guidelines.

  • Develops, coordinates, and communicates the patient's plan of care based on ongoing assessment of the patient and input from the patient's family.

  • Uses clinical expertise to collaborate with multi-disciplinary health care team to improve patient outcomes.

  • Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.

  • Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan.

  • Recognizes symptoms, establishes priorities and responds to patients changing needs throughout the transplant care continuum.

  • Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent in a timely manner per UNOS requirements.

  • Coordinates and schedules all living donor transplant surgeries as assigned.

  • Attends and participates in transplant QAPI meetings, listing meetings, department meetings, multidisciplinary planning meetings and other meetings as needed.

  • Provides cross coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned.

  • Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides, support and feedback

  • Demonstrates a commitment to UI Health's values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment.

  • Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide "quality" patient care as measured by established nursing quality standards and other governing/accrediting health care bodies.

  • Recognizes and supports Shared Leadership as the selected method of nursing governance.

  • Participates in organizational, departmental and shared leadership activities; supports department and unit goals in daily behavior.

  • Collaborates with other nurses and physicians to identify, promote and provide positive learning experiences for staff and students.

  • Initiates, conducts and analyzes evidence-based practice research to improve patient care outcomes. Serves as preceptor, mentor and resource to other nursing staff and students.

  • Other Duties as Assigned

    Minimum Qualifications

    1. High school diploma or equivalent.
    2. Graduation from an accredited school of nursing.
    3. Eligibility for registration with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
    4. Two years of nursing experience in the particular branch of service to which assigned, one year of which involved work comparable to a Clinical Nursing Consultant I.
    • Ability to select appropriate teaching methods based upon the educational and experience backgrounds of employees.
    • BSN (or higher) required of an accredited nursing program.
    • Current BLS/CPR and/or other required unit specific certifications
    • Current advanced cardiac life support, or within 6 months of hire for main hospital.
    • Nationally certified through ANCC approved certifier, or within 6 months of hire in an area relevant to their clinical area.


    Preferred Qualifications
    • BSN (or higher) required of an accredited nursing program.
    • Current BLS/CPR and/or other required unit specific certifications
    • Current advanced cardiac life support, or within 6 months of hire for main hospital.
    • Nationally certified through ANCC approved certifier, or within 6 months of hire in an area relevant to their clinical area.
    • Previous RN experience working with liver transplant patients preferred
