WHAT YOU'LL DO As a Care Coordinator, you'll be the connective thread that keeps clients engaged in their care from first contact through successful treatment. You will: - Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources - Follow up proactively with clients to track progress and troubleshoot barriers to care - Coordinate with primary care teams, behavioral health providers, and community partners on appointments and treatment plans - Educate clients and families on care continuity and available resources - Document client progress and service engagement in electronic health records - Help clients navigate healthcare and social service systems - Participate in interdisciplinary team meetings and ongoing trainings WHAT YOU BRING - High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field - Minimum age of 21 - Valid driver's license, reliable transportation, and proof of auto insurance - Strong communication skills and the ability to build rapport with diverse clients - Comfort with electronic health records (EHR) and Microsoft Office - A collaborative, adaptable, and empathetic approach to client care FULL BENEFITS PACKAGE - Generous vacation, sick, personal, and holiday leave - Employee Assistance Program (EAP) and fitness reimbursement - Health, dental, and vision insurance with substantial employer contribution - Flexible spending accounts (healthcare and dependent care) - Additional life insurance options - 401(k) and Roth retirement plans - Tuition assistance and continuing education opportunities - NHSC loan forgiveness eligibility Ready to make a difference?
