BHC Alhambra Hospitalis located in the beautiful city of Rosemead, California. Our 115 bed facility established in 1924 prides itself on upholding a standard of excellence that allows our patients to take the first step to recovery. BHC Alhambra Hospital is a private, fully accredited, full-service acute psychiatric hospital serving children, adolescents and adults. BHC Alhambra is dedicated to providing quality behavioral health, substance abuse, and eating disorder treatment. We offer inpatient, residential, partial hospitalization and intensive outpatient options for individuals in the communities of the San Gabriel Valley, Los Angeles County, and surrounding areas.

TheCase Manageris responsible for managing the treatment activities offered to the patient, which may include the appropriate clinical screening of potential admissions, establishment of treatment plans, interface with the treatment team/external case managers/managed care organizations and implementation and supervision and/or accomplishes all aspects of the discharge planning process.This position will also include traveling off-site to our Reasons Eating Disorder Centers locations in Pasadena and Arcadia.

Key Responsibilities include:

  • Participate in the implementation of the treatment plan and conduct an ongoing review of the plan to ensure effective implementation of all treatment interventions.
  • Maintain ongoing direct contact with the attending practitioner, treatment coordinator, and various members of the team.
  • Assist in ensuring that the patient attends the appropriate level of care or program as indicated on the treatment plan or as needed when there are changes in the patient's status.
  • Interface with the staff of the appropriate level of care to facilitate a smooth transition at the time of transfer.
  • Maintain necessary documentation of all services and the quality of care.
  • Assure proper tracking of all reviews
  • Completing PsychoSocial Assessments.
  • Working with mental health and eating disorder patients.

Qualifications

Requirements:

Education:Masters degree from an accredited college or university in social work, mental health, psychology or a related field.

Experience:A minimum of four (4) years direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, family motivation, treatment planning and communication with external review organizations is preferred. Strong written, verbal and interpersonal communication skills are required.
