THE POSITION

DESCRIPTION OF WORK

This position plays a key role in supporting older adults by identifying needs, arranging services, and ensuring timely intervention. The work focuses on helping individuals access resources that improve safety, health, and independence. As an Aging Care Manager 3, you will perform the following duties:

  • Case Reviews: Review categories of need and begin investigations within required time frames according to regulations of the Older Adults Protective Services Act
  • Risk Reduction: Reduce or mitigate risk to older adults and complete investigations within regulated timelines
  • Service Planning: Develop care plans, complete assessments, and document contacts using the SAMS program
  • Resource Coordination: Arrange medical, psychological, and social assessments and connect individuals with appropriate services
  • Community Outreach: Provide education on protective services and mandatory reporting through public presentations
  • Team Collaboration: Meet with supervisors, attend network meetings, and assist with related task force activities

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
  • Travel, overtime and after hours may be required.
  • Telework: There is the possibility of some work from home hours after a probationary period.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

  • You must be able to perform essential job functions.

Legal Requirements:

  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
