Gulf Coast Medical Center Care Management is seeking an experienced Medical Social Worker for PRN shifts8am - 4:30pm on a PRN "as needed" basis.

Qualified candidates will have experience working with high risk or vulnerable populations of Care Management experience. Acute care experience strongly preferred.

Gulf Coast is a comprehensive stroke center that also provides care to patients for specialties such as Oncology, Cardiology, Orthopedic and Trauma. This opportunity allows you to utilize your MSW skills in an acute care setting to make an impact in the lives of our patients.

Summary

The Social Work practice that occurs in hospitals and other health care settings inspires health, and aids physically ill patients and their families. The Social Worker assists in resolving the social and psychological problems related to the illness. The Social worker sensitizes other health care providers about the social and psychological aspects of illness. The Social Worker empowers patients to be proactive with regard to their health and personal well-being.

The Social Worker provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high-risk populations. The Social Worker will link patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning. The Social Worker has the opportunity to provide Field Instruction for MSW students. The Social Worker provides counseling and support to patients and families regarding adjustments to disease process and illnesses. The Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning, and conduct screenings for CBT resource linkage.

Requirements

Education:

Masters Degree in Social Work required.

Experience:

Minimum of 1-year clinical experience in medical or healthcare setting.

License:

Other:

Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task and prioritize responsibilities.
