Position Overview: The Trauma-Focused Community-Based Services Care Coordinator will provide services to children and youth who have experienced trauma, with a specific focus on those at risk of or involved in Commercial Sexual Exploitation of Children and other types of sexual abuse. The Care Coordinator will provide home and community-based services under the supervision of a clinician using evidence-based and trauma-informed therapeutic interventions to address the complex emotional, psychological, and behavioral needs of young people who have experienced trauma. Working collaboratively with families, schools, community organizations, and other service providers, the Care Coordinator will support holistic recovery and resilience in the youth and their families.

Essential Functions:

  • Under the supervision of a clinician, deliver trauma-informed and culturally competent individual, group, and family services to children and youth with the goals of reducing high-risk behaviors and the risk of placement disruption.
  • Provide psychoeducation to caregivers with the goal of increasing knowledge relating to the potential impacts of sexual abuse on the survivor and family.
  • Address the specific needs of youth impacted by CSEC, including trauma, complex PTSD, disrupted attachment, anxiety, depression, and other behavioral health challenges.
  • Collaborate with multidisciplinary teams, including clinicians, peer mentors, medical professionals, and educators, to provide holistic support to youth.
  • Participate in the 24/7/365 on-call rotation to provide crisis intervention and support as needed.
  • Maintain timely and accurate documentation in the TFS Electronic Health Record system, ensuring compliance with organizational and regulatory standards.
  • Complete all required program assessments including pre and post-tests.
  • Provide ongoing risk assessment, safety planning, and stabilization for youth during critical situations.
  • Assist families with resource development as needed.
  • Support the implementation and evaluation of individualized recovery goals.
  • Support the aftercare and discharge planning process, in collaboration with the youth and all providers, to include referrals to relevant aftercare supports as appropriate.
  • Participate in all required consultation and training to uphold a specialized skillset.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a related field. Required
  • Valid driver's license and reliable transportation. Required
  • Bilingual (English/Spanish)-Preferred
  • A minimum of three years of experience providing trauma-informed care.

Knowledge, Skills, and Abilities:

  • Experience working with at-risk youth and families preferred.
  • Experience with foster care/families formed through adoption.
  • Strong understanding of family dynamics, youth development, and evidence-based therapeutic approaches.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work collaboratively within a multidisciplinary team and with external partners.
  • Flexible and adaptable to providing culturally competent and client-centered care.

Salary Range: $40,000-$50,000
