Job Description

Job Title: Diabetes Program Manager

Department: Clinical Operations /Diabetes Management Center

FLSA Status: Exempt

Reports To:Director of Clinical Services

General Summary/Overview:

Responsible for developing/teaching educational classes related to weight management. Performs in accordance with the facility*s policies and procedures. Follows the facility*s standards for ethical business conduct. Conducts self as a positive role model and team member. Participates in facility committees meetings in-services and activities.

Principal Duties and Responsibilities:

1.Plans yearly schedule of classes and participates in marketing campaign including making presentations speaking on TV/radio shows and at other speaking opportunities.

2.Develops weight management classes and related materials.

3.Assesses lifestyle eating habits of individual patients. Determines appropriate diet/dietary restrictions.

4.Teaches classes individualizing as appropriate.

5.Follows up with patients with phone calls including problem-solving and positive reinforcement.

6.Documents patient progress for medical records.

7.Collects/analyzes statistics on program attendance patient results and recommends changes.

8.Other duties as assigned

Qualifications:

BS in nutrition food service health or related degree; completion of American Dietetic Association (ADA) examination for registered dietitian (RD).

Minimum of three years' experience dietitian in health field; minimum one year experience in instructional planning and delivery.

Current registration with Commission on Dietetic Registration (CDR).

Knowledge of nutrition principles and application to weight management. Knowledge of how to plan curriculum and use adult learning instructional techniques. Knowledge of lifestyle assessments and dietary considerations.

Skills and Abilities Required:

Skill in accurately assessing patient weight and lifestyle factors and appropriately determining appropriate calorie level and diet. Skill in developing/delivering classes in enthusiastic motivational manner. Skill in participating effectively as member of interdisciplinary team interacting appropriately with physicians nurses other health educators.

Ability to communicate and work effectively with variety of patients families staff publics. Is able to speak persuasively in front of groups with professional manner/image. Ability to use office and media equipment to develop/deliver programs. Ability to help patients* problem-solve about making appropriate lifestyle changes.

Working Conditions:

While performing the duties of this job the employee is frequently required to walk; stand; reach with hands and arms; climb or balance; stoop kneel crouch or crawl; and talk or hear. The employee is occasionally required to sit. May be exposed to various patient conditions including infectious diseases blood and bodily fluids.
