Compensation Range:

Anticipated Base compensation rate is in the range of $203,550.00 - $230,378.00 / Year depending on qualifications and experience.

Expectations:

The Director, Case Management is responsible for overseeing the Case Management department. This role provides direct supervision to department managers. The Director is tasked with driving continuous improvement of hospital systems, ensuring patients have timely access to the most appropriate, safe, and patient-centered care across the continuum. The Director aligns departmental activities with the hospital's strategic goals, using evidence-based care coordination strategies to refine processes and optimize resource allocation. This position requires flexibility in hours and a strong commitment to enhancing patient outcomes through efficient care coordination.

EDUCATION:
Required:
Bachelor's degree in Nursing.

Preferred:
Master's Degree in Business Administration, Healthcare Administration, Public Health or equivalent
Accredited or certification of Case Manager (ACM/CCM)

EXPERIENCE/TRAINING:
Minimum 5 years of management experience in case management within a hospital setting is required.

LICENSES/CERTIFICATIONS:
Required:
Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
